Function

The largest cuneiform, supporting the first ray's base; its wedge is broad on the plantar side, unlike the other two cuneiforms.

  • Wedge shape — broad plantar surface (unlike the other cuneiforms)

  • Intermediate cuneiform
  • First and second metatarsals

Clinical relevance

It is part of the tarsometatarsal (Lisfranc) joint complex; the Lisfranc ligament runs from it to the second metatarsal base — missed injuries here are disabling.
