Long head: supraglenoid tubercle of the scapula (intra-articular tendon). Short head: coracoid process.
Insertion
Radial tuberosity of the radius and bicipital aponeurosis into the forearm fascia.
Action
Flexes the elbow and is the most powerful supinator of the forearm (turning a screwdriver clockwise with the right hand). Weakly flexes the shoulder via the long head.
Nerve supply
Musculocutaneous nerve (C5–C6).
Blood supply
Brachial artery branches.
Clinical relevance
Long-head tendinopathy causes anterior shoulder pain in overhead athletes; distal biceps ruptures pop during heavy lifting and reattach surgically to restore supination strength. The biceps reflex tests C5–C6.
