Origin

Ischial spine and sacrospinous ligament.

Insertion

Lateral border of the sacrum and coccyx.

Action

Draws the coccyx forward and completes the pelvic diaphragm's posterior wall.

Nerve supply

S4–S5 branches.

Blood supply

Inferior gluteal artery.

Clinical relevance

With the levator ani it closes the pelvic outlet; sacrospinous ligament fixation sutures pass through its ligament in prolapse surgery.
