Origin

Body of the pubis, tendinous arch of the levator ani, and ischial spine.

Insertion

Perineal body, coccyx, anococcygeal ligament and the contralateral muscle — forming the pelvic diaphragm's sling.

Action

Supports the pelvic organs, maintains continence, and forms the voluntary sphincter mechanism of the anorectum with the puborectalis.

Nerve supply

Pudendal nerve (S2–S4) and direct sacral branches.

Blood supply

Inferior gluteal and pudendal arteries.

Clinical relevance

Vaginal childbirth stretches it — the muscle behind pelvic organ prolapse and stress incontinence; puborectalis release treats obstructed defecation.
