Base of the great toe's distal phalanx.
Action
Extends the great toe and dorsiflexes the ankle — the muscle tested at the big toe for L5 radiculopathy.

Deep peroneal nerve (L5).
Blood supply
Anterior tibial artery.
Clinical relevance
Big-toe extension weakness localises L5 root lesions — a localising gem of the neurological exam.
