Origin

Posterior tibia below the soleal line.

Insertion

Bases of the distal phalanges of the lateral four toes.

Action

Flexes the toes and plantarflexes the ankle; supports the medial arch and the toe-off push.

Nerve supply

Tibial nerve (L5–S1).

Blood supply

Posterior tibial artery.

Clinical relevance

Its tendon is transferred to replace a failed tibialis posterior in flatfoot reconstruction; imbalance with the intrinsic foot muscles produces claw toes.
